can licensed counsellor use H0004/H0031 code for behavior health counselling or psychotherapy? or they have to use 99384-99387 codes?

H0004 and H0031 are generally payer specific. Medicaid uses these codes as do some commercial payers. You'd need to check.

Is H0004 used for the actual initial visit? Then also bill H0031 for an assessment such as a CFAR test?
