Dear All,

I have doubt from the below OT notes from our Hospital. I coded 29846(TFCC Repair Wrist) and 25295 (Tenolysis Wrist) but insurance denied 25295 saying part of procedure. I searched many sites but i am not able to get the information. Kindly give me some insights on this.

Patient placed in supine on the operating table and applied tourniquet
around the upper arm. After draping the right wrist positioned for routine
arthroscopic procedure. A physical examination of the wrist revealed no
clinical instability. A diagnostic arthroscopy performed through two dorsal portals.
Hypertrophied synoviom treated with partial synovectomy using soft tissue ablator and 3.5mm shaver
using RF device. Open FCU tenolysis done. TFC chondroplasty done
using soft tissue ablator and 2.5mm shaver. Debridement of chondral flap tear
done using shaver 3.5mm and RF device.

Best guess...the 29846 also states "joint debridement" I would imagine they are considering the tenolysis as local debridement......just because it was done "open" does not mean it is not local...it is still same body part/area...."local" debridement is included in many codes.
