Incision was carried out over the ulnar nerve. Subcutaneous tissue was divided. Hemostasis was obtained. We did decompress the cubital tunnel. We then insufflated the joint and established a medial portal and lateral portal and began our lateral capsular takedown stripping the capsule off the anterior aspect of the humerus and shaving some osteophytes and then debriding this all the way to visible brachialis. We then switched viewing portals and completed our division of it with the backbiter taking care to protect the neurovascular structures. We removed a very large loose body from the anterior joint and improved his extension significantly to within 15 degrees. A posterior portal was established followed by posterolateral portal and then anatomy was almost unrecognizable. We worked quite a bit in the posterolateral gutter and removed significant osteophyte disease and then once identifying the olecranon fossa, cord out the olecranon fossa with a burr and shaved off osteophytes both posterolaterally and laterally as well as straight posterior along the olecranon. We then shaved the capsule circumferentially and stripped it off the posterior aspect of the humerus.
