Is the application of a long leg moulded club foot cast (CPT 29450) at the end of a percutaneous Achilles tenotomy (CPT 27606) to correct Clubfoot deformity separately billable? There is not an NCCI edit between these two codes but I keep going back to the CPT book - According to CPT guidelines, cast application or strapping (including removal) is only reported as a replacement procedure or when the cast application or strapping is an initial service performed without a restorative treatment or procedure.
 
If I am reading this correctly the achilles tenotomy was performed to treat the club-foot. If so, then no as a restorative procedure would have been performed before casting.
