I have a Medicare pt who had a L mastectomy for lobular carcinoma. A few years later, she had a prophylactic R mastectomy, with no evidence of cancer. She had bilateral reconstruction with implants, and she's now wanting both implants replaced or revised. I know MC will cover the L one for the hx of CA, but what about the R?
