Hi. I'm working on a denial and the insurance is asking for an "alternative level 2 code for L3260." Any ideas?
 
I have come across this many times with my podiatrist that I code for. It states in an LCD for Orthopedic Footwear, that according to a national policy determination, a shoe and related modification, inserts, and heel/sole replacements, are covered only when the shoe is an integral part of a brace. These are impossible to get paid for my Medicare.
 
Thank you for responding. That does make sense to me now since it was a Medicare patient. I just left the code as is and resubmitted it & it hasn't come back since.
 
Try resubmitting it with clinical notes to why they dispensed the surgical boot. We got a few paid that way. If the patient has a secondary, then submit to the patients secondary.
