Which CPT codes would you use when the patient has an administration in both arms of Cimzia(J0718) for RA?

we bill the Cimzia J0718 x 400
and 96401
For our medicare contractor (palmetto GBA) we use a 50 and 59 and double the price on the SQ injection. They deny if X 2 units.

For other payers a 50 modifier x 2 units.
