PLEASE HELP WITH CODING FOR ADMINISTRATION OF CHEMO.

Docetaxel IV G: 55 mg
55 mg (25 mg/m2; BSA: 2.24 m2 = 56 mg) intravenously Piggyback once. Admin over: 60
minutes Admix fluid: 0.9 % Sodium Chloride Volume: 250 mL to 500 mL Instructions: Protect
from bright light. Store in protective packaging.
Use only non-DEHP products for administration.

Carboplatin IV G: 300 mg
300 mg (2 AUC; CrCl Method: Cockroft-Gault - Actual Body Weight, S.Creatinine: 0.7 mg/dL,
Calculated Creatinine Clearance: 125 mL/min Capped at 125 from 161.101 mL/min)
intravenously Piggyback once. Admin over: 30 minutes to 60 minutes Volume: 100 mL to
500 mL Instructions: Mix in either D5W or NS. Carboplatin is a vascular irritant.

PREMEDICATIONS
Ondansetron IV G: 8 mg
8 mg intravenously Piggyback once. Admin over: 20 minutes
Dexamethasone IV G: 10 mg
10 mg intravenously Piggyback once. Admin over: 20 minutes Admix fluid: 0.9 % Sodium
Chloride Volume: 50 mL
Diphenhydramine IV G: 12.5 mg
12.5 mg intravenously Push once. Admin over: 2 minutes
Famotidine IV G: 20 mg
20 mg intravenously Push once. Admin over: 2 minutes
