Wiki Abdominal myomectomy, polypectomy, ovarian cyst

Hello, can someone help with coding the following:
Abdominal myomectomy
Endometrial polyp removal
Left ovarian cystectomy

What is stumping us is the polypectomy. The myomectomy was done abdominally and the polyp was removed while in. Cannot seem to find a code that would cover the polyp removal in this setting.

Your help would be appreciated.

I don't think there is a code for an open endometrial polypectomy. I would code 58140 or 58146 (depending on number and/or weight of the myomas) for the myomectomy and 58925 for the ovarian cystectomy. I would consider adding -22 to the myomectomy for the polypectomy if I felt the op note justified that. The alternative is unlisted, but I wouldn't want to have the entire claim pended/denied due to that small portion.
