If a provider sees the patient on 9/13 and increases her medication dose, the patient calls back on 9/18 having hallucinations because of the increase, can the provider bill for an audio call? They called the pharmacy and spent some time going over her med list and does not document the how long they spoke to the patient and how long they spoke to the pharmacy, they did document 21-30 minutes.
 
No. The telephone E/M code could not be reported for a call within 7 days of a related office visit. This is included in the instructions and code descriptors for the telephone service. I don't think this is a billable service. Documentation of 21-30 minutes is wouldn't support time attributable to services such as principal care management that would require a more specific time statement. Cindy
