Hello I need a clarification regarding annual physical charts . Suppose we have a patient who is coming for office visits and we already have billed 99213 i.e. subsequent visits for the previous chart and today patient is coming first time for annual physicals so we have to bill 99386 Initial preventive service code or 99396 Subsequent preventive code ?
 
If the patient has been seen by your office within the past 3 years then you would need to use CPT 99396. As the patient would be considered an established patient.
