For pulmonary rehabilitation, should 97110 be billed with G0237 to allow for the exercise required to get the heart rate up to do the breathing therapy/exercises. If so, does it need modifier 59?

Modifier 59 only needs to be used when your combing codes 97110 (Therapeutic Exercise) with 97530 (Therapeutic Activity). When your using G0237 with 97110 you don't need to use a modifier at all.
