Wiki 93965 upper and lower limbs same day

Our vascular lab is performing 93965 on both bilateral upper and lower extremities on the same day. They are billing 93965 & 93965-76. Medicare MUE is 1. The code description is not clear if it includes all extremities.

Does anyone know if 93965 includes both sets of extremities or if we can actually bill for both on the same day?

We are receiving denials from Medicare.

I think you need to use modifier -59 in this scenario.

93965 and 93965-59

Our medicare carrier does not allow modifier 59 when two of the same cpt are performed on the same date. Only 76.

Okay that is how our Medicare provider has us code them.
