Humana denied this claim stating it is a non-covered charge. I have done some reserach and I came accross and article saying you must code with Z98.890 or Z98.891 in order for insurance to cover this. Has anyone else run into this issue?
 
I have been running into this issue recently. I add Z98.890 if the EGD & Colonoscopy were done prior to the Capsule, which is a requirement for most insurances & noted in the LCD L34081. I am not sure what Z98.891 Hx of uterine scar has to do with it?? Do you do the new CapsoCams?
