I see a notation under this cpt code in the book that says,"for laparoscopic approach, 51992". Does this mean that 57288 is an open procedure? If so, where can I find that information in writing?

Yes, 57288 is open. CPT guidelines define procedures that are not identified as laparoscopic, to be open procedures, unless listed otherwise.
