Previous day, pt had atrial septal defect repair. She subsequently developed coagulopathy & hemorrhage shock.
Earlier in the day, surgeon took her to the OR & performed exploratory laparotomy, control of liver hemorrhage w/ packing & VAC placement.
This surgery in question:
Abdominal wall had some bleeder which were controlled w/ electrocautery.
Sponge of VAC & abd reexplored.
Oopherectomy was done.
Liver packs were then removed & revious hemostatic attempt was re-evaluated. Medial aspect of the subcapsular tear still oozing w/ clots. Clots removed. Surface of liver was cauterized & new packs of Surgicel & Fibrillar were placed. New lap pads were applied.
Replaced a new ABThera VAC.
Surgeon has a quantity 2 of 47362 (acutally, he chose 47361 x 2. Am I missing something with the liver x 2?
