When billing for screening is it okay, to use only Z12.11 or does it require a primary dx?

Yes you would just use Z12.11

If the primary reason the patient is having a colonoscopy is for a cancer screening, Z12.11 is the correct code.

Sometimes, providers will state a patient is having a screening when experiencing symptoms, and the colonoscopy is performed to find the problem. This is a diagnostic procedure and not a screening, as the patient needs to be asymptomatic for it to be a screening.
