37211 is arterial and 37212 is venous. Do you choose the code by access or final placement? Dr Z Health lists 37211.

Issue CGS A56460 policy does not allow 37211 arterial or 37212 venous for I26.09. Novitas A55237 allows 37212 venous, with I26.09 but not 37211.
