We billed 21933, 21931, and 21930x2 and are getting denied by Medicare for the second 21930. We should be able to bill all these together with Modifer 59's correct?

21933 does not need the modifier but the other three do. also be certain that these were all separate incisions or they don't qualify for separate reimbursement.
