We have been having issues with Anthem and 20610. The guidelines says to bill J3590 with $0.00 for the biologic content. We have been doing this but they are denying for the NDC number. Has anyone else had this issue and if so, how have you resolved?

We are running into the same thing. I've not gotten a response from Anthem yet. I've attached what they sent to me and I'm sure the rep I spoke with said to use zeros for the NDC # but it doesn't say that in the policy.
