For 2 exostoses removed from the metatarsal-cuneiform joint, one from each bone... 28104 x 2, or just x1?

It does have a MUE of 2 so in theory it can be billed twice but documentation would have to support billing it twice through the same incision. If there was a significant amount of work in performing both excisions then bill twice. Otherwise no.
