I have a patient that had bilateral mastectomies for fibrocystic disease. She needs her implants removed and replaced (one is ruptured). Would you code this as a 19325, because she didn't have breast cancer?

19325 is for the insertion. If the physician is removing the implant I would code 19328 if it is not ruptured and 19330 if it is ruptured.
