If the the pt has Lung cancer and anemia, do we assumed the anemia is due to the cancer and we code if the documentation does not say it is ?

No, there is not an assumed linkage.

The doctor needs to specifically document that the anemia is due to the neoplasm (D63.0) or due to chemotherapy (D64.81).

If the documentation only states "anemia", then you'd have to use D64.9.
