What would be the appropriate pregnancy diagnosis for add on 58611 ?

Of course Z30.2 but Idaho Medicaid always approve fails my billing due to a pregnancy diagnosis needed.

Some of these insurers drive me crazy. Z30.2 is the only diagnosis you would use... patient is no longer pregnant and a sterilization is not a postpartum complication.
 
In FL we are having issues with delivery code being deduced by 50% if a BTL is billed

I am having an issue with one of the Florida MMA's reducing the delivery reimbursement by 50% if we bill for a tubal on the same admission as the delivery. We have appealed the reduction numerous times without success and have not been able to get paid correctly. It is very frustrating. We have opted not to bill for the BTL. We have reported the issue, is anyone else having this same issue?
