Hello,

How should i code ICD-10 for no gestational evidence for an OB ultrasound?

US OB 1st trimester

Need more information. Why was the u/s done?

Chief complaint : O26.90
Findings : Transabdominal and transvaginal images are provided
Impression:
1. No identifiable gestation, either intrauterine or extrauterine.. ectopic pregnancy is not excluded
2. Thickened Endometrium
3. Small amount of fluid in the left adnexa

Well that is very "unspecific" and not very helpful. You may need to dig into this further and find out what pregnancy related condition she had. I mean there has to be a specific reason that she needed to have the u/s. There are so many ways that this could go.

I have the same thing. I am not, in anyway, an OB coder. Mine is slightly different tho. My dr says there is a "porbably corpus luteum right ovary. She is status post intrautrine insemination with abnormally rising beta HCG levels. No evidence of intrauterine pregnancy and no definite ultrasound evidence of ectopc pregnancy. Tailed pregnancy and/or ectopic pregnancy can not be excluded and close follow up is recomended. Any help would be appreciated. Thank you in advance.

Z31.83 may be your only choice here. In the outpatient setting you can't code a "probable" diagnosis as confirmed as you could if she was inpatient and there are no specific code for abnormally rising beta HCG levels.
