Question: How would I code escutcheonectomy? The patient is having scrotoplasty and buried penis repair.

Answer: You will not separately report the escutcheonectomy. Instead, consider it part of the coding you will report for the buried penis repair. The escutcheon is the tissue just above the penis and below the pannus, also known as the mons pubis. Urologists often perform escutcheonectomy during the repair procedure for buried penis.

First, submit 54300 (Plastic operation of penis for straightening of chordee (eg, hypospadias), with or without mobilization of urethra) for the repair of a buried penis. Then, report 55175 (Scrotoplasty; simple) or 55180 (… complicated), if performed, based on your urologist’s documentation.
