Question: What is the correct way to report bipolar electrical vaporization of the prostate, or “button,” TURP?

Answer: The correct code choice hinges on the urologist’s technique when performing the procedure. You should submit 52601 (Transurethral electrosurgical resection of prostate, including control of postoperative bleeding, complete (vasectomy, meatotomy, cystourethroscopy, urethral calibration and/or dilation, and internal urethrotomy are included)) for vaporization of the prostate performed with a button electrode.

For vaporization performed through laser, bill 52648 (Laser vaporization of prostate, including control of postoperative bleeding, complete (vasectomy, meatotomy, cystourethroscopy, urethral calibration and/or dilation, internal urethrotomy and transurethral resection of prostate are included if performed)). 

History: Prostate surgery has undergone many changes in recent years, with different types of prostate surgeries being performed different ways. In your case, a bipolar prostate “button” was used to resect the prostate and vaporization was used to sculpt the prostatic channel. Vaporization of the prostate performed through button should be coded with code 52601. Had the vaporization been performed through laser, code 52648 would had been the code of choice.

The word “vaporization” can lead to confusion since vaporization can be performed through laser as well as button. Verify the exact procedure details from the medical records to avoid any denials.
