Add 74420 to Claim if Insurer will Allow with a Cystoscopy and Retrograde pyelogram

Question: I am coding for left ureteral stent removal, retrograde pyelogram, and ureteroscopy with stone basket extraction. What codes do I report?

Answer: You can submit both 52352 (Cystourethroscopy, with ureteroscopy and/or pyeloscopy; with removal or manipulation of calculus (ureteral catheterization is included)) and 74420 (Urography, retrograde, with or without KUB) if the insurer will cover the 74420. Add modifier 26 (Professional component) to 74420 if the procedure was completed in the OR and your physician does not own the equipment.

Important: The stent removal is included in CPT® code 52352. If the stent is replaced, then report 52332 (Cystourethroscopy, with insertion of indwelling ureteral stent (eg, Gibbons or double J type)), which also includes cystoscopic stent removal.
