Code 74420 Represents Radiological Interpretation

Question: Documentation shows that our urologist performed cystoscopy, bilateral retrogrades, and bimanual rectal/pelvic exam under anesthesia. He also interpreted and documented his reading of the radiological study in the medical records. What is the best way to bill this encounter?

Answer: The most appropriate codes to report are 52005 (Cystourethroscopy, with ureteral catheterization, with or without irrigation, instillation, or ureteropyelography, exclusive of radiologic service) and 74420 (Urography, retrograde, with or without KUB) with modifier 26 (Professional component) appended. You should not bill the pelvic exam separately because it is included in the procedure.
