Make sure you're using the most up-todate version of Medicare's Hospice Quality Reporting Program Quality Measures User's Manual. The Centers for Medicare & Medicaid Services posted a new version of the Hospice Item Set-based manual last month at www.cms.gov/Medicare/Quality-Initiatives-Patient-Assessment-Instruments/Hospice-Quality-Reporting/Downloads/HQRP-QM-Users-Manual-v200_January-2018.pdf.
