Here's How To Fix Your Reason Code 32113 Problem

Watch out for junk data in a Medicare claim field making your claims bounce back.

Problem: Change Request 10155 implemented a new field in the Fiscal Intermediary Standard System (FISS) Direct Data Entry (DDE) entry screen to allow for the reporting of a provider taxonomy code for the attending physician, explains HHH Medicare Administrative Contractor CGS in a message to providers. "The new field, ATT TAXO, is a ten position alphanumeric field that appears on FISS Claim Page 03. Although this information is not required for home health and hospice providers, if invalid information is entered in the ATT TAXO field, your claim will be sent to the return to provider (RTP) file (status/location T B9997) with reason code 32113."

Solution: "If your claim is in RTP with reason code 32113, either enter a valid attending physician taxonomy code or make sure the ATT TAXO field is blank, and press F9 to allow your claim to continue processing," CGS instructs.
