This is the first time I've had this situation and I'd like to get it right. Here is the pertinent dictation.
1445 IV access obtained in left hand with 22 gauge quick cath... Banana bag hung (1L 0.9NS +mvi 10ml, thiamine 100 mg and folic acid 1mg) at 500 cc hr per pump...
1505 Zofran pump in place subcutaneoulsy per needle in right lower abdomen... reprogramming zofran infusion pump to hight rate... increased to 16mg per day... increased to run 1.3ml hr...
1515 Pt dosing in reclining chair, iv infusion banana bag remain at 500 cc hr per pump without incident.
Here's what I have so far
96365 IV infusion, up to 1 hr
96366 addtl hour (1L at 500cc/hr would take 2 hours)
96368 concurrent infusion
?? Reprogramming of pump
J7030 Infusion, NS (sodium chloride) 5000cc
??? Banana Bag ???
I have queried the physician regarding start and stop times, as it apprears that 1L was infused at 500 cc per hour which would take 2 hours.
In reviewing prior OV notes, it appears that the Zofran pump was initiated several days prior to this visit. During this visit the pump was reprogrammed.
