Does anyone know if these diagnoses codes should go in the Assessment of the Primary Care Doctor visit notes . He is ordering these procedures, e.g. the mammo is sent to the imaging place and the stool test is sent to the labs (e.g. quest, labcorp). I mentioned to them that these diagnoses codes are used by these places who are doing the tests. Am I correct ? or are they done by the primary care doctor (I don't think it is the latter).
