Our providers sometimes do well check ups with asthma checks or well check ups with ADHD checks. In ICD-10 should I use encounter for routine child health examination with abnormal finding or without abnormal finding? We already know they have these diagnosis. Please give your thoughts.

An abnormal finding would be if the provider discovers something not quite right at the well exam. So even though the patient has pre existing conditions, if the patient presents with no complaints or concerns, yet the provider discovers something not in a normal range at the exam then it is an abnormal finding. If the patient presents for a well visit but utters a symptom/complaint, then the exclude 1 note tells you you cannot code these together, you must code only the symptoms and defer the well visit.
