Wiki X-ray documentation

I work in a Orthopeadic office and I have a few physicians that when the patient comes in for a follow up visit in there x-ray documentation they want to state the following. "Previous office x-rays and report reviewed" I don't have a issue with this but I am trying to get them to aleast state the findings. (Example...Preivous office x-ray and report reviewed. Showing Slater Harris fracture) Am I right in asking this? Or is what they are documenting enough? Can anyone point me in a direction that I can find some documentation to support either way.

if they give you the date of the x ray report you can code the findings from the report. Physician and outpatient coders can code from the radiology report as the radiologist is a physician that has rendered a dx.
