Wiki Wrist pain with tenosynovitis, synovitis

I am looking for help coding these procedures:

1. Posterior interosseous nerve neurectomy, distal forearm.
2. Extensor tenosynovectomy.
3. Excision, dorsal ganglion capsular degenerative type formation.
4. Arthrotomy with synovectomy of proximal and midcarpal rows.

I know 64772 but am unsure of which other codes to use.
