In the History portion of the report, MRI read as Non displaced occult fractures of the capitate and base of 3rd and 4th metacarpals.

Patient comes to us and the Impression: Occult fracture of the left wrist

Do I code the capitate and metacarpals fractures or do I code the Wrist Fracture

If I code the wrist fracture what cpt code do I use (I could not find one for just wrist fracture - closed treatment)

If I code the capitate and the metacarpals do I code 2 separate cpt codes?

If you look up wrist fracture in ICD it takes you to carpal bones, so I would code the fracture of the capitate. Whether you use more than one CPT really depends on the insurance company. Medicare will only pay for one fracture in the same anatomic area if the fractures are treated with the same cast, brace etc (documentation found NCCI edit policy manual)
