If a provider documents pre and post debridement measurements and documents that 50% of the wound was debrided, do you use the post debridement measurements or pre debridement measurements to calculate the percentage? If you have any links or reference material that speaks to this specific question that would be awesome.

Debridement codes are based on size measured in cm and tissue depth: Skin/subq, muscle/fascia and bone. All of these need to be documented for each wound debrided.
