First question: If the CRNP admits from the office and later the doctor sees the patient at the hospital but the doctor does not dictate anything in addition to the original plan, can you bill under the doctor? Perhaps the physician may write something in the chart, but there is not any dictated information on the H&P report. I know that you can bill "shared" on the documentation for the services of the doc and CRNP together, but not sure if the doc does not elaborate anything in addition to the information provided by the CRNP.


Second Question: If the CRNP admits from the office and the doctor does not see the patient until the next day, do you bill for the H&P under the CRNP for the day of the admit?

If the NP has their own number to bill under, I would bill his/her note --- depending on the documentation the visit may qualify as a 99215 -- you cannot bill shared service unless the physician documents that he or she reviewed the NP's note, and additionally documents at least one of the three compnents of the E&M visit.

as for the DOS, if the NP sees patient on thursday and admits to hospital, but MD see's patient friday, bill the date the patient was actually seen and for the office visit since the service was rendered in the office.
