Wiki Who can bill the fee?

I am researching the specifics regarding who gets to bill for the handling fee when a specimen is sent to an outside lab. And does the type of specimen ever come into play, i.e., blood versus a pap smear. I need as much variable supporting documentation for either outcome.

Providers office delivers specimen to lab.
Specimen is picked up by the outside lab.
