Wiki Who bills for intubation performed by an RT?

For an intubation procedure performed by an RT under MD supervision, should the MD bill for the procedure (CPT 31500), with the RT listed as 'service provider' on the claim? I could not find an NPI# for the RT, and could not find any guidance on this.
 
I don't think you can bill a professional charge for this because payers do not credential RTs and it would most likely not meet the definition of an 'incident to' service. Where did the service take place and who employs the RT? If this is was done in the hospital and the RT is a hospital employee, then the facility should be able to include a charge for the RT service on the hospital UB claim.
