Patient is in a facility that is located inside of a hospital. The patient is discharged from our facility to another floor of the hospital. So on the same date, there will be a discharge and an admission. And no, the same physician is not doing the admit as the discharge. So my question is: Where does the -25 modifier go in this situation? On the admit? discharge? both? I would appreciate any input. Thanks.
