Wiki When does the H&P start for a Hospitalist??

Does it begin when the Hospitalist Service assumes care for the patient on their service? ie - at 11:15 pm on 03/10, or when the physician actually sees the patient ie- 1:00 am on 03/11.

If the Hospitialists initial visit is on 03/11 - then that is the date that they would bill their Intial Hospital visit. If the patient was actually seen on 03/10, then that would be the date. The Hospitalist cannot bill for a service on a date that did not take place.

That was my thinking as well, but the hospitalists went to a webinar and came back with the idea of billing the H&P the evening before when the patient was actually "accepted" by their service.

They reason that the physician may be reviewing the chart or ordering tests before the patient gets to the floor after midnight to be examined.

But I really think all the criteria of the H&P needs to be done, including gace to face time with the patient.

Any other thoughts???
 
initial visit

99221-99223 are not H&P codes or admit codes, they are initial visit codes. These should not be billed until the physician has seen the patient face-to-face in the hospital. If the patient is admitted on 1/1 but the MD does not see them until 1/2, there would not be a billable encounter for 1/1 and the initial visit code (or whichever code is appropriate for the type of service) would be billed on 1/2.
