Patient has prostate cancer. Has prostatectomy. When do you switch from coding this as "prostate ca" to "history of ca" ? Thanks for your help!

It becomes hx of when there is no tx being rendered for the primary site and there is no evidence of disease at the primary site. If the patient is still recieving chemo or radiation or is on Lupron then the ca is active.

It would also be active cancer if the patient elects to receive no treatment and it is clearly documented. If there is evidence of disease then it is current, no evidence of disease AND not undergoing treatment it's Hx.
