The physician that is seeing these patients in their homes does not have an office, he works for various home health agencies. With what should we be populating box 32 (Service facility location? The address of the Home health agency?
 
I don't have any experience with Home Health billing, so I may be wrong, but I believe if you use Place of Service 12 on your claim, then box 32 should be left blank.
