Has anyone cracked the code for billing home telehealth visits to commercial Anthem and receiving payment?
I have been working on a number of test claims and they are all failing for incorrect coding no matter what combination I use for POS 10 (Telehealth provided in a patient's home) and home/ALF E&M codes. I have tried with and without a 95 modifier.
When I sent POS 10, CPT 99350 (as an example) I get a denial due to "Missing/incomplete/invalid/inappropriate place of service."
When I sent POS 10, CPT 99350, modifier 95 I get the denial "The procedure code is inconsistent with the modifier used or a required modifier is missing. Note: Refer to the 835Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present. N657 This should be billed with the appropriate code for these services"
