We often get notes from our providers that state CVA as the patient diagnosis. When do we use the history of V12.54 and when do we use the 434.91? thanks!

Hi,
V12.54 would be used if the patient has had a CVA in the past with no residual effects. Usually it will be documented as history of CVA. If the CVA just occurred, then I would use code 434.91. If the documentation states that there are residual effects from the CVA like dysphasia, I would use a Late Effect CVA code.
