Wiki What is the best way to bill for 99213, 95909, and 95885 for a patient that spent a week in SNF?

I have claim that I had a takeback on, unless I use Modifier 26 on 95909 and 95885. Insurance has stated because the patient was in patient at a SNF for a 7 day period while they were seen in our office. Insurance has paid 99213. What is the best way to bill this? Bill the modifier 26? Or, should I bill the SNF for the other 2 codes?
