We are moving forward with a new program called Healthaware and will be scheduling patients for heart screenings and would like to know what level of service should we charge for the following services that will be performed by our Nurse Practitioners. The visit will include weight, height, blood pressure, pulse, BMI, pulse oximetry, nutrition and exercise counseling, and establishing healthy goals. There will be a flat fee charged for this service but we would like to know what E&M level it would be. Also, can Nurse Practitioners see new patients in the clinics?

You might look at code 99420, but I'm not sure of the Nurse Practitioner's role though. Maybe someone else can help you there. We do those assessments at our clinic but we do not bill to commercial insurance. This is a service we provide to our local companies' employees and the company pays. Hope this helps!
