I read somewhere that 4 out of 5 medical bills has at-least some minor error. What are the usual errors that may cause claim denials?

On today's calls, the most common denial error was a bill for a new patient when they've seen the provider within 3 years. The second most common denial reason today was a workman's comp issue with multiple body parts (i.e. different claim numbers) for the same patient evaluated on the same day with an automatic response saying "claim already paid this is an exact duplicate of a claim already paid." The third most common denial was that the bill was forwarded to the Medical Group.

The first and last ones are in-house errors that the front desk should be wary of. The second one is the insurance's issue.
